When coming in for an assessment at Dove Recovery, please bring a valid form of identification, such as a driver's license or passport. Additionally, it is helpful to bring your insurance card and any relevant medical records or information regarding your substance abuse history, past treatments, medications, and any co-occurring medical conditions. Lastly, make a list of all medications you are currently taking, including prescription drugs and over-the-counter medications. Bringing these items will assist our team in conducting a thorough assessment and developing an appropriate treatment plan tailored to your needs.
